Exercise for Brain Health: What the Evidence Says
The organ that benefits most from your workouts may be the one doing none of the work. Here's what the research shows about exercise, memory and the aging brain.
We tend to file exercise under the neck-down benefits: heart, muscles, waistline. The research increasingly points upward. Physical activity is one of the few interventions with credible evidence for protecting cognition as we age — not through any single mechanism, but through a cascade that starts with a raised heart rate and ends, remarkably, with measurable changes in brain structure. Here is what the evidence says.
Does exercise actually improve cognitive function?
For adults over 50, the trial evidence says yes. A systematic review with meta-analysis of 39 randomised controlled trials found that exercise improved cognitive function in people older than 50 regardless of their baseline cognitive status, with both aerobic and resistance training contributing (Northey et al., 2018). These are controlled experiments, not just observations of active people — the improvement follows the training.
Exercise changes the brain's structure
The most famous demonstration is anatomical. In a year-long randomised trial in older adults, aerobic exercise increased the size of the anterior hippocampus — the brain region central to memory formation — by 2%, effectively reversing one to two years of age-related shrinkage, with matching improvements in spatial memory and increases in serum BDNF, a protein that supports neuron growth and survival (Erickson et al., 2011). The control group, doing stretching, continued to show the expected decline. Few interventions of any kind have been shown to physically enlarge a memory structure in an aging human brain.
Does being active lower dementia risk?
The long-range data are consistent. A meta-analysis of 58 prospective studies found physical activity associated with a 20% lower risk of all-cause dementia and a 14% lower risk of Alzheimer's disease — associations that persisted even in studies with more than 20 years of follow-up, which argues against the idea that early, undetected dementia simply makes people inactive (Iso-Markku et al., 2022).
Fitness itself may matter as much as activity. In a 22-year Finnish cohort of more than 2,000 men, each standard-deviation increase in measured VO₂max — maximal oxygen uptake, the standard index of cardiorespiratory fitness — was associated with a 20% lower dementia risk, and men in the lowest fitness quintile had nearly double the risk of the highest (Kurl et al., 2018).
The honest caveats: this is observational evidence, so reverse causation can never be fully excluded, and the landmark FINGER trial — which improved cognition in at-risk older adults — combined exercise with diet, cognitive training and vascular monitoring, so exercise's isolated contribution there can't be separated (Ngandu et al., 2015).
Why intensity earns a place
Several of the proposed mechanisms — BDNF release, improved vascular function, better insulin sensitivity — scale with how hard the effort is, and the fitness–dementia data above suggest the aerobic capacity you build, not just the minutes you log, carries the association (Kurl et al., 2018).
That is where REHIT fits — Reduced Exertion High-Intensity Interval Training, CAROL's signature workout of two 20-second all-out sprints in a roughly five-minute ride. To be precise about the claim: no trial has tested REHIT against dementia or memory outcomes. The chain is inferential — REHIT raises VO₂max by 12–15% in six weeks in controlled trials (Metcalfe et al., 2012), and higher VO₂max is associated with lower dementia risk and better brain aging (Kurl et al., 2018). Each link is evidenced; the end-to-end trial has not been run. We'd rather tell you that than imply otherwise.
The bottom line
Exercise is one of the best-supported things you can do for your brain: randomised trials show cognitive improvement in over-50s (Northey et al., 2018) and structural growth in the hippocampus (Erickson et al., 2011), while decades-long cohorts link both activity and measured fitness to substantially lower dementia risk (Iso-Markku et al., 2022). The prescription is unglamorous: raise your fitness, keep it raised, and start before you need to. Your brain is keeping score either way.
